Excuse Forms

Please use the following forms to submit an excuse for your student's absence within three (3) days of their absence, or it will remain unexcused.

Additional Information:

• Please know that if your child's absence excuse exceeds three days, it will not be marked as excused. 

• If more than one child in your family is absent, please fill out a form for each child.
